Bridge Direct Care provides all of the primary care services that you need. We are available when and where you need us, and we serve patients of all ages.
Our membership model means that you will have access to primary care like you’ve never experienced before. Your monthly membership fee covers high quality comprehensive care with no added fees including:
Unlimited office visits for primary care services
Access by phone, email, text, and video chat
Relaxed 30- to 60-minute appointments
Same-day or next-day appointments with minimal to no wait times
No co-pays
Discounted medications, imaging, and lab fees
No surprise costs ever
Monthly Membership Fees:
There is a one-time enrollment fee of $100 for individuals and $200 for households.
Family Includes two adults and one child, $35 for each additional child.
We offer low income & hardship pricing - inquire if needed.
Unfortunately we are not able to accept Medicare patients at this time.
